The District V Forensic Diagnostic center serves a thirteen county area in north central Ohio.

District V Forensic Diagnostic Center | The District V Forensic Diagnostic Center is currently one of ten Forensic Evaluation Centers in the state of Ohio. We are affiliated with Catalyst Life Services, which is located in Mansfield, Ohio.

The District V Forensic Diagnostic Center serves Ashland, Crawford, Hardin, Holmes, Huron, Knox, Lorain, Marion, Morrow, Richland, Seneca, Wayne, and Wyandot counties in north central Ohio.

What We Do | The Forensic Diagnostic Center provides forensic evaluation services to the criminal courts for 13 counties in north central Ohio. We also provide consultation and training to local criminal justice systems, Alcohol, Drug Addiction, & Mental Health Services (ADAMHS) and Community Mental Health (CMH) boards, and other community mental health agencies. Finally, staff of the District V Forensic Diagnostic Center provide expert testimony to the courts when requested.

Types of Evaluations | The District V Forensic Diagnostic Center provides multiple types of evaluations as needed by common pleas, municipal and juvenile courts. These are performed by psychologists who meet forensic training requirements. Please note that fees may be required depending on the type of evaluation requested and the referring court. All evaluations are considered the property of the referring court and cannot be distributed without that court's permission.

2945.371(G)(3) - Competency to stand trial | This is an evaluation of a defendant's present mental condition and whether he or she understands the nature and objective of the proceedings against him or her and can assist in his or her defense.

2945.371(G)(4) - Mental condition at the time of offense charged (not guilty by reason of insanity: NGRI) | This is an evaluation of a defendant's mental condition at the time of the offense charged and whether, due to a severe mental disease or defect, he or she did not know the wrongfulness of his or her acts charged.

2945.40 - Procedure upon acquittal by reason of insanity (Post-NGRI) | Following an adjudication of not guilty by reason of insanity, this evaluation is to determine if a defendant is a mentally ill person subject to court order or a mentally retarded person subject to institutionalization by court order.

2945.401(D) - Nonsecured status ("second opinion") evaluations | The purpose of these evaluations is to approve nonsecured status or termination of commitment of those committed and adjudicated as incompetent to stand trial unrestorable or not guilty by reason of insanity.

2951.051 - Intervention in lieu of conviction | This evaluation attempts to determine if drug or alcohol use, or mental illness, intellectual disability, or status as a victim of trafficking in persons, was a factor leading to the criminal offense(s).

2152.56 - Juvenile competency | This type of evaluation determines whether a child, due to mental illness, intellectual disability, developmental disability, or otherwise, is incapable of understanding the nature and objective of the proceedings or of assisting in the child's defense.

2152.12 ("Rule 30") - Transfer of juvenile to adult court | This evaluates a child to determine amenability to juvenile rehabilitation and determination of factors in favor of, or against, transfer.

2947.06 - Mitigation of penalty | This evaluates a defendant for the trial court to determine the disposition of a case. 

2951.03 - Presentence investigation reports of those guilty of a felony | These are evaluations of a defendant adjudicated guilty of a felony as requested by an officer and, subsequently, the court.

Additional Evaluations | Other referral questions, such as competency to waive Miranda rights, competency to plead guilty, etc., can be addressed on an individual basis.
